Attention Franklin County Residents
Franklin County Volunteers in Medicine Clinic receives a patient assistance grant through the Health and Wellness Trust Fund. This grant assists patients without health insurance or less than optimal coverage to afford their medications.
Prescription Assistance means we (Franklin County Volunteers in Medicine Clinic) will do all the research to see if the medication can be obtained free or at a reduced rate. Patient income guidelines and eligibility requirements for this program are determined by each pharmaceutical company individually. Depending upon the particular medication, some patients even with insurance may qualify for some medication assistance.
If eligible, this service is provided to you for a $2.00 processing fee per medication, per year.
The process for the Patient Assistance Program:
- Patient seen at FCVIMC or at their provider’s office. (Note: not necessary to be a patient of FCVIM clinic)
- Prescriptions are written legibly by the provider
- Referral to FCVIMC for prescription assistance eligibility. Patients call FCVIMC at 919-496-0495 for appointment with Prescription Eligibility coordinator. FCVIMC is open Tuesday – Friday, 9 am to 5 pm. Eligibility appointments can be made at any time during these hours.
- If the patient is eligible and medication(s) are available, the Prescription Assistance Coordinator will print out the forms for patient to complete and sign. The provider’s signature is required. If not a patient at FCVIM clinic, patient must return the completed forms to their medical provider for signature.
- Once the application is signed by both the patient and provider and all required documentation has been produced, the medication is ordered. (This process usually takes 6 to 8 weeks to complete)
- When the medication is received at FCVIMC, a Licensed Pharmacist will label and do intake of the medication for a second level of safety and quality control.
- Patient receives a call from the coordinator. Medication can be picked up ONLY by the patient.
- Medication pick-up is on Tuesday and Thursday from 12- 4 pm. Arrangements can also be made to pick up at the regularly scheduled office visit.
Important Notice to Patients
Patient and provider agree to sign all applications in a timely manner
Patient and provider will notify Franklin County Volunteers in Medicine Clinic Prescription Assistance Program immediately if any of the following changes occur:
- Dose change
- Frequency change
- Discontinuation of medication
- Any other different instructions from the provider
